We are seeking a seasoned professional to join our Business Excellence team in Cannon Hill. As a Client Intake and Assessment Practitioner, you will be the first point of contact for Home and Community enquiries, playing a pivotal role in supporting clients to achieve their goals through access to Endeavour Foundation services.
About the RoleThis is an exciting opportunity to work with a dedicated team that shares your passion for empowering individuals with disabilities. You will be responsible for analysing disability assessments, conducting screenings, and creating tailored support plans to ensure clients are matched with the right services based on their needs and funding.
In this varied role, you will:
- Understand and assess clients' support needs, confirming functional capability and ensuring staff and leaders have appropriate training to meet specific needs.
- SUPPORT A CULTURE OF PERSON-CENTRED PRACTICE THAT PUTS CLIENTS, FAMILIES, CARERS, AND GUARDIANS AT THE FOREFRONT OF EVERYTHING WE DO.
- Provide a customer experience aligned with Endeavour Foundation's Values and behaviours, managing day-to-day relationships with clients, their families, and external referral organisations.
- Foster collaboration with regional site leadership teams, supporting the co-creating and co-delivery approach across and within the business.
- Through a comprehensive understanding of the NDIS and disability sector, match clients to life-changing services that support their goals and aspirations, maximising Individual NDIS funding levels.
- Assist in developing a relevant pipeline of qualified leads aligned to drive growth and sustainability of our core customer program, helping identify new market opportunities.
We're seeking a candidate with a deep understanding of the NDIS and disability sector, who excels at building strong relationships and developing meaningful connections. Your ability to analyse and interpret complex disability assessments allows you to create comprehensive risk assessments, safety plans, and support strategies that meet individual client needs.
You will also possess:
- A degree in Social Work, Psychology, Human Services or a relevant field.
- Minimum 4 years' experience in the community services sector.
- Experience analysing support needs and creating comprehensive assessments to maximise NDIS funding opportunities.
- LARGE VOLUME CASE MANAGEMENT FOR DISABILITY OR SUPPORT NETWORKS IS HIGHLY DESIRABLE.
- Excellent relationship management, interpersonal, and communication skills with an engaging and inspiring communication style to leverage further opportunities.
- Strong analytical skills with the ability to collect, analyse, and disseminate data to drive customer conversion ratios and provide actionable insights for key stakeholders.
- HOLD OR BE WILLING TO OBTAIN A WORKING WITH CHILDREN CHECK (BLUE CARD), NDIS WORKER SCREENING CHECK (YELLOW CARD), NATIONAL CRIMINAL HISTORY CHECK (FIT2WORK), AND AN OPEN DRIVERS LICENSE WITH COMPREHENSIVE INSURANCE ON YOUR VEHICLE.
The successful candidate will receive a salary package up to $15,900 tax-free, plus additional benefits including Salary Packaging Benefits Package, Employee Assistance Program, Corporate Health Insurance plans, discounts with corporate partners, paid parental leave, and more.
