Our Opportunity
We invite an experienced Care Transition Specialist to join our Better Futures team. This role involves engaging in therapeutic relationships with young people to empower them towards independence, focusing on essential life areas: Housing and Living Skills, Education, Employment, Health and Wellbeing, and Community Connections.
The successful candidate will be guided by the Better Futures Framework of Advantage Thinking, supporting youth transitioning from care, encouraging active participation in transition planning, and providing individualised supports both in-care and post-care.
About You
As a skilled Care Transition Specialist, you will possess:
- A strong desire to deliver positive outcomes for young people
- Demonstrated knowledge, skills, and experience in trauma-informed principles and approaches
- Confidence in recommending and implementing new procedures and processes
- Strong organisational skills to execute administrative requirements
- Highly developed cultural awareness and sensitivity, paired with excellent communication skills, to build strong relationships with stakeholders
- Alignment to Cafs' values of Respect, Integrity, Collaboration, Kindness & Innovation

Salary
$85,284 - $91,746 pro rata + superannuation, based on part-time hours and experience.
