About the Role:

We're seeking a compassionate professional to lead our homelessness case management initiative. This role involves providing targeted support to individuals at risk of homelessness, fostering independence and empowering them to secure safe and affordable housing.

Key responsibilities include developing personalized case plans, offering practical and personal assistance, conducting assessments, and connecting participants with specialist services.

Mandatory Requirements:
  • Certificate IV qualifications in Community Services or similar.

  • A genuine passion for making a positive impact in the lives of others.
  • Proven ability to build and maintain strong relationships with diverse stakeholders.
  • Capacity to identify and implement effective solutions.
  • Experience working within fast-paced environments with high-performance expectations.
  • Excellent time management and organizational skills.
  • A strong communicator able to influence a wide range of people.
Other Requirements:
  • Ability to obtain necessary certifications and clearances.
Daily Responsibilities:
  1. Develop and review personalized case plans to meet participant needs.
  2. Provide crisis budgeting, information, advice, and community referrals.
  3. Conduct initial and ongoing assessments of participant needs.
  4. Maximise participants' capacity for independent living.
  5. Advocate and assist participants to access housing and specialist services.
