About this role

We are seeking a skilled and compassionate Case Manager to provide support to individuals experiencing homelessness. As a key member of our team, you will work closely with participants to develop and implement personalized case management plans.

Your responsibilities
  • Develop and implement individualized case management plans in consultation with participants
  • Provide crisis intervention, budgeting advice, and referrals to community services as needed
  • Conduct ongoing assessments of participant needs, including risk assessments and safety planning
  • Support participants to access housing, healthcare, and other essential services
About you

To be successful in this role, you will need:

  • Certificate IV qualifications in Community Services or similar
  • A proven track record of building relationships with stakeholders
  • Strong problem-solving skills and ability to identify solutions
  • Excellent communication and interpersonal skills
