As a Homelessness Case Manager, you will provide case management support to people who are homeless, at imminent risk of homelessness, or housed but at risk of homelessness.

What will a typical day look like?
  1. Work closely with participants to develop, action, and review case management plans to meet their individual needs.
  2. Provide practical and personal support, crisis budgeting, information, advice, and community referrals.
  3. Conduct initial and ongoing assessments of participant needs, including risk assessments and safety planning as needed.
  4. Provide respectful, proactive, persistent, reliable, practical, and comprehensive support that upholds the dignity and privacy of individuals.
  5. Maximise participants' capacity for independent living.
  6. Advocate and assist participants to access housing and specialist services such as drug or alcohol, mental health, legal, domestic, and family violence services.
Mandatory Requirements:
  • Certificate IV qualifications in Community Services or similar.

  • A passion and genuine desire to make a difference in the lives of Central Queenslanders.
  • Proven ability to build and maintain relationships with various stakeholders.
  • Ability to identify and implement solutions.
  • Experience working within a high-performance framework with accountability for KPIs on program delivery and outcomes, in a fast-paced, solutions-focused environment.
  • Effective time management and organizational skills.
  • A strong communicator, able to quickly build rapport and influence a diverse range of people.
Other Requirements:
  • Ability to obtain a Blue Card (working with children check positive notice or exemption).
  • Ability to obtain a Yellow Card (disability services positive notice or exemption notice).
