Job Opportunity

We are seeking a dedicated and passionate Transition Practitioner to join our team. As a key member of our organisation, you will play a vital role in supporting young adults leaving care to achieve their goals and live independently.

About the Role:

The Transition Practitioner provides case management services to ensure young adults receive tailored support throughout their transition to independence. This involves effective engagement with clients, their families, and support networks to identify needs and aspirations. You will develop and implement Care Plans led by the young adults, incorporating administration of eligible financial supports.

Key Responsibilities:

  • Deliver services and supports aligned with program models of care.
  • Build client agency and self-efficacy through empowerment, discouraging dependence on supports.
  • Maintain accurate and detailed case notes of client interactions.
  • Lead/participate in internal and external stakeholder and case review meetings.
  • Engage all allocated client cases, ensuring initial outreach home contact and consent.
  • Develop clear goals and Care Plans based on client-led Assessments.
  • Deliver targeted Motivational Interviewing interventions for each client.
  • Deliver family connection and mediation sessions between young people and their caregivers/adult kin.

Requirements:

To be successful in this role, you will possess a Bachelor's Degree or Diploma in Social Work, Human Services, or related field, along with 2-3 years' experience in social services. You must also have at least 1 year of case management experience and demonstrated ability to work in a performance-driven environment. Additionally, you will need a current drivers' licence and flexibility to work outside business hours as needed.
