Young People Support Specialist

At Employment Plus AU, we are seeking a passionate and values-driven Case Manager to join our team. This is an exciting opportunity to make a real difference in the lives of young people aged 16-18 years old.
Job DescriptionAs a Case Manager, you will provide intensive case management to young people transitioning into independent living and self-reliance. Your key responsibilities include:
- Supporting young people in accordance with the Youth Service Model of Care
- Building effective relationships with clients, encouraging self-determination and participation
- Ongoing assessment of client issues, needs, and circumstances from initial intake to exit
- Ensuring clients have safe accommodation, on-site supervision, guidance, and support across all life domains
- Providing clients with constructive advice on budgeting, shopping, cooking/nutrition, household cleaning, building relationships with neighbors and other community members
To be successful in this role, you will require:
- Certificate IV in Youth & Family Intervention or equivalent
- Degree qualification in social work, welfare, community development, or related field
- Relevant experience in a social service environment
- Psychological Assessment (in line with Children and Young People Safety Act)
- Through their Eyes (Reporting Abuse and Neglect) Training
- First Aid Certificate
- A current Working with Children's Check
- A willingness to undergo a National Police check
