Key Role Information

This role is crucial in supporting and advocating for individuals within the community who are experiencing homelessness or at risk of homelessness. The successful candidate will be working collaboratively with internal teams, external service providers, and community organisations to facilitate access to housing, healthcare, employment opportunities, and other essential resources.

Job Responsibilities
  • Culturally Appropriate Case Management: Provide case management and support services to people and their families experiencing homelessness.
  • Client Relationship Building: Develop strong, trusting relationships with clients to understand their needs and goals, and provide tailored support to help them achieve stability.
  • Collaboration and Partnership: Work collaboratively with internal teams, external service providers, and community organisations to facilitate access to housing, healthcare, employment opportunities, and other essential resources.
  • Client Advocacy: Advocate on behalf of clients to ensure they receive fair and equitable treatment when accessing services and support.
  • Comprehensive Assessments: Conduct comprehensive assessments of clients' needs and develop individualised care plans with achievable goals that empower clients towards self-sufficiency.
