Aboriginal Case Manager Role

This is a challenging and rewarding role that requires a dedicated individual to support and advocate for individuals within the community who are experiencing homelessness or at risk of homelessness.

The Aboriginal Case Manager will provide culturally appropriate case management and support services to First Nations people and their families, helping them to achieve stability and independence.

Key responsibilities include:

  • Providing tailored support to clients to help them achieve their goals and improve their well-being
  • Working collaboratively with internal teams, external service providers, and community organisations to facilitate access to housing, healthcare, employment opportunities, and other essential resources
  • Advocating on behalf of clients to ensure they receive fair and equitable treatment when accessing services and support
  • Conducting comprehensive assessments of clients' needs and developing individualised care plans with achievable goals
  • Maintaining accurate and up-to-date case notes, reports, and client records in accordance with organisational guidelines

About You

We are looking for someone with:

  • A Certificate IV in Community Service or other relevant qualification and/or case work experience in an associated field
  • A current Driver's Licence
  • Satisfactory Criminal record check
  • Current Working with Children Check (NSW)
  • Evidence of full COVID19 vaccination
  • Understanding and experience of working with Aboriginal people and their communities
  • Demonstrated experience in case planning and coordination of people with complex support needs
  • Understanding of, and demonstrated ability to work with Human Service Agencies
  • Demonstrated experience and/or understanding of Trauma Informed Care
  • Excellent communication skills, particularly in the areas of negotiation, facilitation, mediation and problem solving
  • Intermediate computer literacy skills including Microsoft office suite and databases
