Job Role
The role of an Intake Case Worker is to provide support and guidance to individuals facing complex challenges in their lives.
This involves assessing their needs, coordinating services, and linking them to external agencies when necessary.
This position offers the opportunity to make a real difference in people's lives by connecting with new and existing clients, addressing their individual needs, and facilitating access to crisis support.
- Provide holistic casework and assessments to address individual needs, manage risks, and facilitate access to crisis support when required.
- Coordinate referrals to internal and external services as and when needed.
- Maintain knowledge of local service networks and deliver short-term interventions aligned with case management practices.
- Support frontline services, including staff and volunteers.
- Ensure accurate client records and reporting.
- Participate in debriefings, outreach, community work, and ongoing training as needed.
- Tertiary qualification in social work, social science, welfare, or related discipline.
- Minimum two years' experience in supporting individuals to navigate 'social housing' providers.
- Working knowledge dealing with individuals with mental health and AOD.
- Referral experience to internal and external providers for further clinical assessment, support, and case management.
- Experience with relevant client information management system.
