Social Services Professional

We are seeking a dedicated professional to provide high-quality social work services to clients with chronic and complex health conditions.
The successful candidate will be responsible for managing a clinical caseload, providing professional level services to clients in accordance with professional codes of ethics and NSW Health Code of Conduct.
- Providing social work services to clients with chronic and complex health conditions.
- Managing a clinical caseload and providing professional level services to clients.
- Working collaboratively with multidisciplinary teams to achieve optimal outcomes for clients.
- Developing and implementing evidence-based practice in social work interventions.
- Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers.
- Current NSW State Driver Licence and willingness to use for work purposes.
- Clinical skills in assessment, planning, and evaluation of social work interventions with adult clients who have chronic and complex health conditions.
- Understanding of the psychosocial impact and management of chronic health conditions, ageing, and disability.
