Job Title: Clinical Care CoordinatorSalary: NZD $80,000 - $100,000 per annum
This is an exciting opportunity to join our team as a Clinical Care Coordinator. As a key member of our multidisciplinary team, you will be responsible for providing comprehensive assessment and support to clients in their homes using interRAI and other computer-based tools.
In this role, you will:
- Act as an expert clinical resource, using advanced assessment skills and appropriate assessment tools to plan and oversee support within a multidisciplinary team.
- Provide comprehensive assessment with the consumer in collaboration with family/whanau and other primary health providers.
- Develop individual service plans in collaboration with the consumer, family, whanau, utilising a determined tool.
- Maintain key relationships with stakeholders; Clients, families/whanau and relevant agencies.
- Deliver services aligned with contractual obligations and Organisation's Policies and Procedures.
You will have:
- A Registered Health Professional qualification (such as a Registered Nurse, Occupational Therapist or Physiotherapist).
- A current annual practicing certificate with the relevant regulatory body.
- Current Drivers licence (Full Licence Class 1).
- Knowledge of working with older people and/or those with disabilities in the community.
- Understanding of relevant Health & Safety and Employment legislation and is able to apply the requirements.
