About This Role

Visionwest Community Trust is seeking a registered health professional to fill the position of Care Manager. This role is part of our multidisciplinary team responsible for providing comprehensive support services to clients in their homes.

Key Responsibilities
  • Act as an expert clinical resource, utilizing advanced assessment skills and appropriate assessment tools to plan and oversee support within a team.
  • Direct and delegate to Support Workers involved in medication administration, ensuring safe and appropriate practices.
  • Provide comprehensive assessments in collaboration with clients, families, and primary healthcare providers.
  • Develop individual service plans in collaboration with clients and their support networks.
  • Coordinate clinical requirements, including client reviews, skill mix, and Support Worker competencies.
  • Maintain key relationships with stakeholders, including clients, families, and relevant agencies.
  • Deliver services aligned with contractual obligations and organizational policies.
  • Undertake administrative tasks associated with the role.
Requirements
  • Registered Health Professional (e.g., Nurse, Occupational Therapist, Physiotherapist).
  • Current practicing certificate with relevant regulatory bodies.
  • Current Full Licence Class 1 driver's license.
  • Knowledge of working with older adults and/or those with disabilities in the community.
  • Understanding of relevant Health & Safety and Employment legislation.
  • Excellent interpersonal, team building, analytical, and problem-solving skills.
  • Competency in working with human services databases and intermediate Microsoft Word skills.
  • Knowledge of the local area.
