Home Health Clinical Coordinator

We are seeking a skilled Home Health Clinical Coordinator to join our team at Visionwest Community Trust. This role is ideal for a registered health professional who is passionate about delivering high-quality care in a community setting.
Key Responsibilities- Provide expert clinical guidance and support to multidisciplinary teams, utilizing advanced assessment skills and tools to ensure effective care planning.
- Direct and delegate responsibilities to Support Workers, ensuring safe administration of medication and optimal client outcomes.
- Conduct comprehensive assessments in collaboration with clients, families, and primary health providers, identifying individual needs and developing personalized service plans.
- Coordinate clinical requirements, including client reviews, skill mix, and competencies for Support Workers, to ensure they are equipped to deliver services effectively.
- Maintain strong relationships with stakeholders, including clients, families, and relevant agencies, to ensure seamless care delivery.
- Deliver services aligned with contractual obligations and organizational policies, upholding the highest standards of quality and safety.
- Registered Health Professional (e.g., Registered Nurse, Occupational Therapist, or Physiotherapist) with current annual practicing certificate.
- Current Driver's License (Full Licence Class 1)
- Knowledge of working with older people and/or those with disabilities in the community.
- Understanding of relevant Health & Safety and Employment legislation, with ability to apply requirements.
- Excellent interpersonal, team building, analytical, and problem-solving skills.

Salary and BenefitsThe estimated salary for this role is $80,000 - $100,000 per annum, depending on experience and qualifications.
