This is a full-time position with an estimated salary of $80,000-$110,000 per annum. The role is based in Waikato - Hamilton City.

Job Description

We are seeking a Registered Health Professional to lead our clinical team as a Care Manager. In this role, you will be responsible for:

  • Acting as an expert clinical resource using advanced assessment skills and appropriate assessment tools to plan and oversee support within a multidisciplinary team.
  • Directing and delegating Support Workers involved with medication administration, ensuring safe and appropriate practice.
  • Providing comprehensive assessments in collaboration with clients, family/whānau, and other primary health providers.
  • Developing individual service plans in collaboration with consumers, family, whānau, and utilising designated tools.
  • Coordinating clinical requirements, including client reviews, skill mix, and competencies for Support Workers.
  • Maintaining key relationships with stakeholders, including clients, families/whānau, and relevant agencies.
  • Delivering services aligned with contractual obligations and Organisation's Policies and Procedures.
Required Skills and Qualifications

To be successful in this role, you will need:

  • A current annual practicing certificate with the relevant regulatory body.
  • A valid Full Licence Class 1 drivers' licence.
  • Knowledge of working with older people and/or those with disabilities in the community.
  • Excellent interpersonal skills, team building skills, analytical and problem-solving skills.
  • Competence in working with human services databases and intermediate-level proficiency with Microsoft Word.
