About This Role

We are seeking a highly skilled clinician to join our team in delivering high-quality care services to our clients.

Job Summary

The successful candidate will be responsible for designing and developing restorative care services, delivering evidence-based clinical support, empowering clients and their support networks, maintaining clinical and program knowledge, and collaborating with multidisciplinary teams.

This is an exciting opportunity for an experienced clinician to contribute to the development of innovative service models and make a meaningful impact on the lives of our clients.

Key Responsibilities
  • Design and Develop Restorative Care Services: Lead and contribute to the development, rollout, and continuous improvement of the Restorative Care Pathway under Support at Home, collaborating with internal teams and sector partners to co-design innovative service models.
  • Deliver Evidence-Based Clinical Support: Conduct functional and clinical assessments, develop personalized goal and exit plans, and coordinate therapeutic interventions through our partner network to support client wellness and reablement.
  • Empower Clients and Their Support Networks: Educate and support clients, families, and helpers with practical strategies to enhance independence and quality of life at home.
  • Maintain Clinical and Program Knowledge: Stay informed about the Restorative Care Pathway program requirements, including support plan monitoring and funding optimization, while maintaining accurate and timely clinical records.
  • Collaborate and Drive Continuous Improvement: Work closely with multidisciplinary teams to ensure coordinated, person-centered care and actively contribute to service development, quality improvement, and professional learning initiatives.
Requirements

To be successful in this role, you will need:

  • Tertiary qualification in physiotherapy or occupational therapy or another allied health discipline.
  • Demonstrated knowledge, understanding, and experience in key areas such as current aged care reforms, the Support at Home program, familiarity with the Aged Care Quality Standards, restorative care and reablement approaches, current registration with AHPRA, police clearance certificate or willingness to apply, current driver's license, and excellent interpersonal and communication skills.
