Job Title: Restorative Care Specialist

This role involves delivering high-quality, person-centred care to individuals. As a Restorative Care Specialist, you will contribute to the development of innovative service models and coordinate therapeutic interventions to support member wellness and reablement.

The key responsibilities of this position include:

  • Service Development: Lead and contribute to the design, rollout, and continuous improvement of restorative care pathways, collaborating with internal teams and sector partners to co-design effective service models.
  • Clinical Support: Conduct functional and clinical assessments, develop personalised goal plans, and coordinate therapeutic interventions through partner networks to support member reablement.
  • Member Empowerment: Educate and support members, families, and carers with practical strategies to enhance independence and quality of life at home.
  • Clinical Knowledge: Stay informed about restorative care program requirements, including support plan monitoring and funding optimisation, while maintaining accurate and timely clinical records.
  • Team Collaboration: Work closely with multidisciplinary teams to ensure coordinated, person-centred care and actively contribute to service development, quality improvement, and professional learning initiatives.

Requirements:

  • Qualifications: A tertiary qualification in physiotherapy, occupational therapy, or another allied health discipline.
  • Experience: Demonstrated knowledge and experience in aged care reforms, restorative care approaches, and current registration with relevant authorities.
  • Skills: Excellent interpersonal and communication skills, including the ability to build rapport with individuals and their families, and work collaboratively as part of a team.
