Community Support Program Coordinator

We are seeking a dedicated Community Support Program Coordinator to join our team in delivering community social support programs.
Key Responsibilities:- Facilitate and evaluate various social support programs, including community and outing-based activities.
- Provide client-centred support, assisting clients with daily living needs and communication, while observing and supporting their well-being.
- Organise client transport to and from venues, using personal vehicles and community buses as needed.
- Coordinate meal preparation and service, ensuring safety and quality standards.
- Participate in improvement initiatives, attending relevant training and team meetings.
- Sustain a safe environment for clients and staff by adhering to safety policies and performing safety audits as required.
- You hold a Certificate IV in Leisure and Health, Ageing Support, Community Services, or equivalent.
- You have experience working with elderly clients and individuals with disabilities in a community care setting.
- You possess a strong understanding of client-centred programming, CHSP, and HACC PYP programs.
- You are knowledgeable about Food Safety and Occupational Health & Safety standards.
- You have strong communication skills and the ability to work collaboratively in a team.
- You are basic computer literate in Microsoft Office applications.
- You hold a valid Victorian Driver's Licence, a reliable personal vehicle (with comprehensive insurance), and the ability to drive a Council's 12-seater bus.
- You hold current First Aid and Food Handling Certificates (or willingness to obtain).
- You have a current Police Check and Working with Children's Check.
- Bilingual skills in a community language are highly desirable.
- A competitive hourly rate of $39.89 per hour.
