Acute Care Specialist

The Geriatric Medicine Registrar will provide acute care to patients in their homes, working under the supervision of a geriatrician.
Key Responsibilities:
- Deliver optimal patient care as part of multi-disciplinary teams.
- Coordinate patient care and supervise learning opportunities for healthcare professionals.
- Attend ward rounds, case conferences, family meetings, outpatient clinics, and unit meetings.
- Liaise with multidisciplinary teams to deliver patient-centered care.
- Complete accurate and timely discharge summaries.
Requirements:
- Be a registered medical practitioner with a valid Australian Health Practitioner Regulation Agency (AHPRA) registration.
- Hold a current Victorian Driver's license.
- Have relevant training and experience in Geriatric Medicine and General Medicine.
- Demonstrate a commitment to high-quality patient care.
- Exhibit teamwork and collaboration skills.
- Ability to communicate effectively at all levels.
- Current National Police and Working with Children History Check.
