Job Description

Making a difference in the lives of people in sub-acute hospital care is a rewarding career goal.

The social work team provides essential support to patients and their families, working across acute, sub-acute and community based services.

This position offers opportunities to gain experience working in a well-supported multidisciplinary team environment with patients in the sub-acute hospital setting. Work areas include Geriatric Evaluation and Management (GEM), inpatient rehabilitation and community therapy services.

  • Conduct comprehensive assessments and provide emotional support and counseling to patients and their families.
  • Liaise with service providers and refer patients to Community Services, Aged Care Assessment Service and Transition Care Program.
  • Deliver high-quality Social Work services, supervise and educate junior Social Workers, and contribute to a collaborative team.
About You
  • You are a qualified Social Worker with at least 4 years of work experience as a Social Worker.
  • You possess excellent communication skills and adaptability, with proven ability to work autonomously in hospital environments.
  • You can deliver exceptional consumer-focused clinical care to patients and their carers.
  • You have experience and skills to mentor and supervise junior staff members.
Key Responsibilities

As a Sub-Acute Hospital Social Worker, your key responsibilities will include:

  1. Providing psychosocial assessments and support to patients and their families.
  2. Liaising with healthcare providers to ensure seamless patient care.
  3. Supervising and educating junior Social Workers.
  4. Contributing to a collaborative team environment.
