Job Opportunity

We are seeking a dedicated and compassionate individual to join our team as a Care Partner.

The ideal candidate will possess excellent assessment skills, enabling them to identify changing needs and develop tailored plans that empower clients to live confidently and comfortably.

Responsibilities include conducting comprehensive wellbeing assessments, managing client feedback, and collaborating with colleagues to achieve optimal outcomes for our customers.

About the Role

The Care Partner serves as a trusted advisor, delivering high-level service to support and empower clients in making informed decisions and accessing services and funding that enable them to remain in their homes.

This role involves working closely with clients, families, and our multidisciplinary team to respond swiftly to changing needs and ensure seamless operations.

Required Skills and Qualifications

We require a committed and enthusiastic individual who embodies qualities of kindness, energy, and adaptability.

Relevant tertiary qualifications in Human or Behavioural Sciences, Allied Health, Aged Care, or Community Services Fields are essential.

Experience in client case management and a National Police check are also required.
