As a **Client Wellbeing Practitioner (Social Worker)**, you'll play a vital role in enhancing the emotional and psychological wellbeing of individuals living with complex neurological and neurodegenerative conditions.

Working as part of a collaborative multidisciplinary team, you'll deliver direct clinical services to clients experiencing conditions such as Parkinson's Disease, Stroke, and other related illnesses. You'll provide tailored wellbeing management and counselling, while also offering advocacy, clinical assessments, and active referrals. With a client-centred approach, you'll support individuals, families, and carers in navigating the challenges of neurodegenerative illness and mental health.

This is a compassionate, hands-on role that offers the opportunity to make a meaningful difference in people's lives. You'll work under supervision, contribute to the professional development of volunteers and students, and liaise with a broad network of healthcare professionals and community organisations to ensure holistic support.

We're looking for individuals who possess:

- Bachelor of Social Work (or equivalent) and registration with the Australian Association of Social Workers (AASW).
- Demonstrated experience (including clinical placements) in the mental health and/or disability field.
- A sound understanding of the challenges faced by individuals with neurological and neurodegenerative conditions, such as Parkinson's Disease and Stroke - highly regarded.
- Proven ability to work both independently and collaboratively within a multidisciplinary team environment.
- Experience delivering evidence-based therapeutic interventions tailored to individuals with complex neurological conditions.
- Strong interpersonal, communication, and problem-solving skills, with the ability to engage effectively with clients, families, and community stakeholders.
- Confident in facilitating educational sessions and community presentations to raise awareness and support.

**About the role**:
Reporting to the Allied Health Lead, the Client Wellbeing Practitioner (Social Worker) will provide support to clients of THRFG Parkinson's and Stroke. This role includes:

- Deliver individual wellbeing counselling, support, and coaching to people living with Parkinson's, Stroke, and other neurological conditions, as well as their families and carers.
- Conduct client assessments, develop personalised support plans, and review progress to enhance mental health and overall wellbeing.
- Facilitate referrals to relevant community services and supports, and advocate on behalf of clients as needed.
- Maintain accurate records and data reporting in line with funder requirements and organisational standards.
- Lead or support therapeutic and educational group programs, both in-person and via telehealth, across metropolitan and regional areas.
- Develop and deliver community education sessions to improve awareness and understanding of neurological conditions.
- Collaborate with internal multidisciplinary teams and external stakeholders, including health professionals and community organisations.
- Support client engagement with the NDIS, including assistance with planning and implementation of support plans.
- Participate in marketing, referral activities, and community networking events to increase awareness of available services.
- Seek supervision and mentorship from senior staff for complex clinical decision-making and continuous professional growth.
