The Role

We are seeking a dedicated individual to support our Balcatta team as a Team Facilitator.

This part-time permanent role combines direct customer support with leadership and coordination responsibilities. You will work closely with Support Workers and the Service Coordinator to ensure customers receive safe, consistent, and high-quality services.

The Job

You will be supporting customers with complex needs, including spinal cord injury, cerebral palsy, and Parkinson's disease. Duties may include:

  • Bowel care (nurse assessment)
  • Spinal cord injury education and skin care management
  • Catheter care and catheter flush (nurse assessment)
  • Education on Glyceryl Trinitrate (GTN) and transdermal patches
  • Managing autonomic dysreflexia
  • Blood pressure monitoring
  • Personal care, domestic assistance, mental health support

Working alongside customers with challenging behaviors, using Positive Behaviour Support (PBS) strategies and training, you will build strong relationships with customers, their families, and the support team.

Training, buddy shifts, and access to clinical guidance will be provided before taking on complex tasks independently.

What You Need

A valid driver's licence and reliable vehicle

NDIS Worker Screening Check (or willingness to obtain)

Working with Children Check (or willingness to obtain)

Current First Aid & CPR Certificate III in Disability or Aged Care (or working towards)

Experience as a Support Worker, ideally with complex/high-intensity care

Ability to lead, motivate, and support a team
