Key Responsibilities
  • Develop digital marketing and promotional materials across various social media platforms to raise awareness within the community and home care sectors.
  • Build strong partnerships with stakeholders through effective stakeholder management skills.
  • Report directly to the Client Liaison Officer, providing exceptional service to clients.
  • Identify and promote services and programs to external stakeholders, driving new business opportunities.
  • Work closely with the care team, liaising with clients, family members, staff, and a wide range of stakeholders to ensure services are provided according to client needs and business objectives.

The successful applicant will be self-driven, motivated, and a self-starter, striving to deliver the highest level of service.

Qualifications
  • Building strong connections with clients and their families.
  • Service coordination.
  • Organising support to clients with disabilities and monitoring and reporting on their progress.
  • Referencing clients to agencies that can provide additional help.
  • Assessing client needs and resources for health, welfare, and other facilities and services.
  • Responding promptly to emails, enquiries, and requests from clients, staff, and management.
  • Maintaining accurate records of incidents and feedback as required.
  • Re-organising work priorities to meet the needs of a fast-paced environment.
  • Supporting all clients to meet their goals.
  • Conducting Client Care Assessments and Care Planning.
  • Supervision and management of Care Staff.
  • Discussing and promoting Right at Home's services with medical professionals and referral sources.

Perth based with some travel required. The preferred candidate will have experience in assessing and planning client needs, a sound understanding of the Aged Care and Disability systems, and strong organisational, administrative, and time management skills.
