About the role and where you will be working:

In this role, you'll design and deliver personalised care plans reflecting each client's aspirations.
You'll oversee budgets, coordinate services, and ensure compassionate, high-quality care that exceeds expectations.
Working closely with clients and stakeholders, you'll create meaningful outcomes that truly enhance everyday lives.

Please note this role is Max Term of 6 months with potential for extension

We'll also be asking you to:

Develop and manage personalised care plans that reflect each customer's needs, goals, and preferences.

Build and maintain strong relationships with clients, families, and key health or community partners.

Oversee budgets and monitor spending to ensure cost-effective, high-quality care delivery.

Regularly review and adjust services to support evolving client goals and changing circumstances.
