Job Title: Client Service ManagerAbout the Role

We are seeking a highly skilled and empathetic professional to join our team as a Client Service Manager. As the primary point of contact for clients, you will provide exceptional customer service and support to ensure they receive the best possible care and support in their own homes.

Your key responsibilities will include:

  • Providing accurate and timely information about available services to clients and their families.
  • Managing and documenting client feedback, addressing concerns in line with our Feedback Policy & Procedure.
  • Conducting regular wellness visits, client contacts, and annual reviews to optimise care and support.
  • Developing care plans aligned with ACAT/NSAF assessments and client goals, collaborating with clinical and allied health teams.
  • Reviewing and updating care plans annually or as care needs change.
  • Maintaining accurate budget planners, updating them as services or funding levels change.

You will be responsible for building strong relationships with clients and their families, working closely with internal teams to meet each client's individual needs. This is a dynamic role that requires a high level of organisation and attention to detail.

Key Skills and Qualifications

  • Evidence of COVID-19 vaccination.
  • Able to obtain a National Police Clearance within 6 months' validity.
  • Successful completion of pre-employment health checks and reference checks.
  • A valid Class C driver's licence.
  • A reliable vehicle with full comprehensive car insurance.
  • Desirable: Certificate IV in Community Service Coordination, Enrolled Nurse (EN) or Registered Nurse (RN) qualification.
