About this role: As a relationship builder and service coordinator, you will establish strong connections with clients and their families to ensure they receive the best possible care and support. Your focus will be on delivering exceptional services that cater to individual needs and preferences.

Main responsibilities:

  • You will act as the primary point of contact for clients and their families, providing timely and accurate information about available services.
  • Manage and document client feedback, addressing concerns in accordance with established policies and procedures.
  • Regularly conduct wellness visits, client contacts, and annual reviews to optimize care and support.
  • You will develop care plans aligned with assessments and client goals, working collaboratively with healthcare professionals and teams.
  • Care plans will be reviewed and updated at least annually or as client needs change.
  • You are responsible for managing client budgets effectively, ensuring appropriate service delivery while preventing deficits or surpluses.
  • Maintain accurate budget planners, updating them as services or funding levels change.
  • Face-to-face visits and client contact will be conducted as per current procedure.

  • Evidence of COVID-19 vaccination.
  • Ability to obtain a National Police Clearance (within 6 months validity).
  • Successful completion of pre-employment health checks and reference checks.
  • A valid driver's license.
  • A reliable vehicle with full comprehensive car insurance.
  • Desirable: Certificate IV in Community Service Coordination or equivalent qualification.
  • Willingness to complete a paid induction program.
