- Develop and maintain strong relationships with clients, their families, and other stakeholders to ensure a holistic approach to care.
- Work collaboratively with the multidisciplinary team to ensure seamless delivery of services and achieve optimal outcomes for clients.
- Conduct regular assessments and reviews to identify client needs and develop individualized care plans.
- Provide emotional support and companionship to clients, promoting their independence and well-being.
- Maintain accurate and up-to-date records of client interactions, progress, and outcomes.
- Understanding of and a passion for working with the elderly.
- Current driver's license.
- A registered, reliable, and comprehensively insured motor vehicle.
- Confidence in using different forms of technology, such as smartphones or tablets.
- A current First Aid and CPR Certificate or willingness to obtain.
- A current National Police Check or willingness to obtain.
- Flexibility in terms of availability, including the ability to work within the hours of 7:00 am - 7:00 pm, Monday to Friday.
- Permanent part-time employment with guaranteed hours depending on your availability.
- Experience variety by visiting clients in their homes within the West Launceston region.
- Preferred availability from Monday to Friday, with Friday availability being a must for this role.
- Benefit from a base hourly rate of $30.61 (plus superannuation, Km's reimbursement, NFP salary packaging).
- Travel allowance of 0.99c per km and paid travel time between services.
- Daily mobile allowance paid when using your own device for work-related purposes.
