Nurse Practitioners form the cornerstone of high-level clinical care delivery, addressing complex and evolving needs in palliative care.

Job Overview:

As a Palliative Care Nurse Practitioner, you will provide expert clinical assessments, implement advanced care plans, and lead initiatives in evidence-based practice across various settings, including residential aged care, hospital-in-the-home (HiTH), and community environments.

  • Conduct comprehensive clinical assessments to inform patient care
  • Develop and execute advanced care plans tailored to individual patient needs
  • Promote quality improvement through data-driven decision-making and mentoring of staff
  • Collaborate with the Community Palliative Care Team (CPCT) to enhance patient outcomes across care environments
Requirements:

To excel in this role, you will require:

  • Registration as a Registered Nurse with the Nursing & Midwifery Board of Australia, including endorsement as a Nurse Practitioner or working towards same as per NMBA Guidelines on endorsement
  • Advanced skills in diagnostics, assessment, referral, and treatment planning
  • Strong background in palliative care models and interdisciplinary teamwork
  • Leadership capabilities focused on education, mentorship, and service development
  • NSW Class C Driver's License
  • A commitment to upholding Calvary's Mission and Values
