Palliative Care Clinical Leader

We are seeking an experienced and dedicated Clinical Nurse Specialist to join our Palliative Care Service.
About the Role:
- Coordinate, lead, and participate in projects related to patient flow and care coordination across services.
- Monitor patient wait times and discharge dates via electronic patient journey boards, identify key 'Waiting for What' (W4W) and patient exit blockers.
- Lead multidisciplinary meetings to ensure timely patient care, advise referrers, and escalate bed blocks.
- Work within a dynamic team to provide quality care to community clients of the Palliative Care Service.
- Provide evidence-based nursing care to diverse populations, including individuals, families, and communities.
- Take a leadership role in nursing and healthcare coordination, offering expert assessments and triage to optimize outcomes, and provide feedback for improvement.
About You:
- You should be currently registered as a Registered Nurse (Division 1) with the Australian Health Professionals Regulation Agency.
- A valid NSW driver's license (Class C or above) is required.
- Relevant post-registration qualifications and at least three years' experience in palliative care are essential.
- Ability to work within an interdisciplinary team, with a commitment to professional development, effective communication, and interpersonal skills is necessary.
- Strong integration of theoretical knowledge, clinical skills, critical thinking, and time management skills is desired.
