Empowering Individuals Through Effective Care Leadership

You will have the opportunity to make a meaningful impact on the lives of older individuals by empowering them to live with dignity, independence, and connection.
Key Responsibilities:- Lead and mentor a multidisciplinary team to deliver person-centred care that promotes wellness and reablement.
- Ensure services meet quality standards and customer requirements while aligning with organisational strategy.
- Champion a restorative, wellness and reablement approach to care, ensuring customer outcomes are central to every decision.
- Build and maintain strong working relationships with community leadership, health providers, and customers/supporters.
- Drive continuous improvement, quality, and risk management processes to enhance service delivery and staff capability.
To be successful in this role, you will require a Bachelor of Nursing tertiary qualification, current registration and practising certificate with AHPRA, proven leadership experience in a community aged care or health service setting, senior case management and/or coordination experience with a strong customer service focus, and excellent knowledge of aged care legislation, standards, and programs.
