About the Role

InFocus Disability Services, a Support Coordination and Plan Management service and part of the Catholic Archdiocese of Brisbane, is seeking a talented professional to join our team providing coordination services to NDIS participants of all ages.

This is a permanent opportunity with a blend of working from home and from our Woolloongabba office. This hybrid arrangement will have on-going support and easy access to the manager and other team members. Some work-related travel will also be required.

The role will see you help participants to understand and implement their plans which align with the individual's goals, support capacity-building and resilience of the participant, and provide links to tailored informal, formal, and mainstream supports.

Key Responsibilities

  • Provide expert and accurate advice and assistance to participants, including discussion and implementation of NDIS plan goals and objectives
  • Work with families and participants to increase their future capacity to independently manage their supports, as required, which may include identifying a range of appropriate services and supports, providing assistance, mentoring, and support to resolve service-delivery related issues, and providing support and assistance to participants and/or their families to ensure budgets align with the use of services
  • Monitor and report participant outcomes, coordinate services, and work within the financial boundaries of each plan
  • Develop and foster strong local networks and knowledge of service providers
  • Other duties as required

Requirements

  • Knowledge of disability services, NDIS, and inclusion principles, as well as a thorough understanding of mainstream and community services, or the ability to quickly acquire knowledge
  • Ability to identify barriers to supports and highly developed skills in working within complex family situations to assist in the achievement of participants' goals
  • Solid understanding of the impact of disability on the individual, as well as their family and stakeholders
  • Well-developed communication and interpersonal skills, including the ability to manage conflict and resolve/mediate as required
  • Ability to work with a high busy caseload of participants, meet deadlines, and prioritize as required
  • Ability to positively represent the organization and connect with, build, and maintain networks/relationships in the community to maximize the use of available resources and enhance positive outcomes
  • The ability to work autonomously by managing your own workload, as well as the ability to contribute to the Support Coordination team
  • Strong technology-based skills, including client information management systems and Microsoft Office suite of products
  • Ability to work both autonomously and collaboratively with team members and external stakeholders to facilitate seamless service to participants

Requirements Continued

  • Relevant qualification and/or equiv. industry knowledge and skills from experience in the sector
  • Blue Card, NDIS Worker Screening Card, and suitable Police Certificate, or ability to acquire
  • Current Australian Driver's License
  • Commitment to the organization's values and philosophy
