We are seeking an experienced Support Coordinator to join our team on a full-time basis. As a Support Coordinator, you will work closely with our clients to provide personalized support and guidance to help them achieve their goals and objectives.

Salary Package

The successful applicant will receive a competitive salary package, estimated to be around $70,000 - $90,000 per annum, depending on experience and qualifications.

Job Description

Key Responsibilities:

  • Coordinate and manage the services and supports in clients NDIS plans
  • Work closely with clients and families to help them navigate the NDIS and access services
  • Build and maintain strong relationships with clients, service providers, and other stakeholders
  • Empower clients by helping them understand their plans and enabling them to take control of their support services
  • Monitor progress, conduct regular reviews, and provide timely updates to the NDIS
  • Be available to clients, especially in times of crisis, and ensure that their needs are met

Required Skills and Qualifications:

  • Proven experience as a Support Coordinator
  • Demonstrated ability to manage and maintain a caseload
  • Strong interpersonal, communication, and problem-solving skills
  • Relevant qualifications in disability services, social work, or related fields (preferred)
