Job Description

At Centacare Brisbane, we are seeking a highly skilled and dedicated individual to join our team as a Support Coordinator.

About the Role

This permanent opportunity will see you working closely with National Disability Insurance Scheme (NDIS) participants of all ages, providing coordination services that empower them to achieve their goals. You will be responsible for helping participants understand and implement their plans, building their capacity and resilience, and connecting them with tailored supports.

Key Responsibilities
  • Provide expert advice and assistance to NDIS participants on plan implementation and goal achievement.
  • Work collaboratively with families and participants to increase their independence in managing their supports, including identifying suitable services and resolving service-delivery issues.
  • Monitor participant outcomes, coordinate services, and manage budgets within financial boundaries.
  • Foster strong local networks and maintain knowledge of service providers.
Required Skills and Qualifications
  • Knowledge of disability services, NDIS, and inclusion principles, with a solid understanding of mainstream and community services.
  • Ability to identify barriers to support and develop high-level skills in working with complex family situations.
  • Strong communication and interpersonal skills, including conflict management and mediation.
  • Capacity to work autonomously and collaboratively with the Support Coordination team.
  • Proficiency in technology-based systems, including client information management systems and Microsoft Office.
Salary and Benefits

We offer a competitive salary range of $70,000 - $85,000 per annum, depending on experience, plus excellent benefits and opportunities for professional growth.
