Healthcare Coordinator

We are seeking a highly skilled National Care Coordinator to join our team at Cocoon SDA Care. This is an exciting opportunity for a Registered Nurse to take on a challenging and rewarding role, providing clinical guidance and support to our care teams.
About the RoleThe National Care Coordinator will be responsible for coordinating care and providing clinical advice to our teams, ensuring the provision of high-quality best practice clinical care to our residents and participants. This role will also provide training and supervision to support staff in responding to complex care needs, and will work closely with our Quality and Compliance Manager to ensure compliance with legislative and statutory requirements.
This is a full-time position based in Eight Mile Plains, offering a competitive salary and flexible work arrangements. The successful candidate will have a strong background in healthcare, excellent communication skills, and the ability to build strong relationships with residents, participants, and support staff.
Key Responsibilities- Provide clinical guidance and support to care teams, including Service Managers, Team Leaders, Registered and Enrolled Nurses, and Disability Support Workers.
- Assist in identifying service gaps and improvements, and lead ongoing continuous improvement initiatives to achieve clinical best practice.
- Communicate effectively with residents, participants, and support staff, building strong relationships and promoting a culture of choice and independence.
- Support the implementation of our clinical governance framework, and provide oversight of High Intensity Support Plans and assessments.
- Conduct resident or participant re-assessments, reviews, and provide ongoing support to participants, workers, and other stakeholders.
- Coordinate clinical incidents, risks, and complaints, and visit residents or participants in their homes to check clinical compliance and quality.
- A current unrestricted Registered Nurse Registration as approved by the Australian Nurses and Midwifery Board.
- A current NDIS Worker Screening Check, NDIS Mandatory Worker Orientation Certificate of Completion, and Working with Children Check (WWCC) clearance within the last five years.
- A COVID-19 Infection Control Certificate, current COVID-19 vaccination certificates, First Aid, CPR, and Anaphylaxis Training.
- A valid Driver License and associated Insurance, and 100 points of identification.
