Job Summary

We are seeking a skilled RN Coordinator Community Services to join our team at Western Community Services office in Woodville. As a leader in our aged care team, you will be responsible for coordinating personalized care plans, working collaboratively with Registered Nurses, Community Coordinators, and Home Support Workers. Your expertise in clinical knowledge and leadership skills will enable you to make a meaningful impact on the lives of our clients.

Key Responsibilities
  • Clinical Leadership: Apply your comprehensive clinical knowledge to assess client needs, develop individualized care plans, and provide guidance to the care team.
  • Team Collaboration: Foster a supportive and inclusive team environment, working closely with Registered Nurses, Community Coordinators, and Home Support Workers to deliver exceptional care.
  • Client-Centered Care: Prioritize client-centered care, ensuring that each individual receives personalized attention and support to maintain their independence and dignity.
Requirements
  • Current AHPRA registration
  • Proficient with computer systems
  • 2023 flu vaccination, and both Covid-19 vaccinations and Covid -19 booster
