Chronic Disease Management Specialist

The Department of Justice seeks a skilled Nurse Coordinator to join its health services team in the Western Australian Corrective Services system.
This role focuses on improving the health and wellbeing of incarcerated individuals with chronic conditions, such as diabetes, cardiovascular disease, respiratory illnesses, and other long-term conditions.
Key Responsibilities:- Clinical leadership in care coordination and oversight
- Comprehensive assessments and development of individual care plans
- SUPPORT for self-management through education and culturally appropriate health promotion
- Liaising with prison health services staff, general practitioners, mental health and allied health teams, and custodial staff
- Contribution to discharge planning and continuity of care
To be successful in this role, you will need:
- Eligibility for registration in the category of Registered Nurse by the Nursing and Midwifery Board of Australia
- An Australian citizenship or permanent resident status
- A validCorC-Aclass driver's license or equivalent
