Care Coordinator Role

At our organisation, we require an experienced and passionate Care Coordinator to join our team. As a key member of the care team, you will be responsible for delivering high-quality in-home care and assistance to our clients.

The successful applicant will be working with a wide range of stakeholders to ensure that services are provided in accordance with client needs, goals, and preferences. This includes assessment, planning, and reviewing care plans as required.

You will also be responsible for building strong connections with clients and their families, assigning staff to clients based on qualifications and availability, and organising support to clients with disabilities.

">

Other key responsibilities include referring clients to agencies that can provide additional help, assessing client needs and resources, responding promptly to emails and enquiries, and maintaining accurate records of incidents and feedback.

We are seeking someone with experience in assessing and planning care needs, a sound understanding of the Aged Care and Disability systems, and a strong client focus.

Preferred skills include demonstrated experience managing staff, current National Police Check, Working with Children Check, Full Driver's Licence, and the ability to work in a fast-paced environment.

Key responsibilities:

  • Maintaining accurate records of incidents and feedback as required">
  • Re-organising work priorities to meet the needs of a fast-paced environment">
  • Supporting all clients to meet their goals">
  • Conducting Client Care Assessments and Care Planning">
  • Supervision and management of Care Staff">
  • Discussing and promoting Right at Homes services with medical professionals and referral sources">
  • Reporting to head office">
