Care Coordinator Role

We are seeking a Care Coordinator who is motivated, reliable, and passionate about delivering high-quality care services.

This role involves providing quality in-home care and assistance to our fast-growing client base under various funding sources, including private pay clients, My Aged Care, and NDIS.

The successful applicant will work closely with our care team, liaising with clients, family members, staff, and stakeholders to ensure services are provided in accordance with client needs, goals, and preferences.

Key responsibilities include:

  • Building strong relationships with clients and their families
  • Assigning staff to clients based on staff qualifications and availability, as well as client preferences
  • Organizing support for clients with disabilities and monitoring their progress
  • Referring clients to agencies that can provide additional help
  • Assessing client needs and resources for health, welfare, and other facilities and services
  • Responding promptly to emails, enquiries, and requests from clients, staff, and management
  • Maintaining accurate records of incidents and feedback as required

In addition to case management, this role will involve managing inside and outside sales enquiries, developing and maintaining new and existing referral sources.

To succeed in this role, you will need:

  • Experienced in assessing and planning care needs
  • A sound understanding of the Aged Care and Disability systems
  • A strong client focus based on their goals, preferences, and needs
  • Demonstrated experience managing staff
  • Current National Police Check
  • Working with Children Check
  • Full Drivers Licence
  • All candidates must have the right to work in Australia
  • Organisational, administrative, and time management skills
