About the Role:

As a skilled caregiver, you will provide comprehensive, client-centered support in accordance with each client's personalized care plan.

You will play a pivotal role in assisting clients with daily living activities and ensuring their comfort in their own homes.

Key Responsibilities:

  • Provide personal care and assist with daily tasks, adhering to the Client Care Plan.
  • Collaborate closely with Registered Nurses to deliver client-specific care, including personal hygiene, toileting, nutrition, mobility, and more.
  • Identify client care needs and work collaboratively with the client's care team to ensure high-quality, holistic care.
  • Communicate effectively with clients, their families, and team members.
  • Support clients with light housekeeping duties to promote independence, including laundry, cleaning, and meal preparation.
  • Maintain accurate daily client notes and promptly report any concerns to the RN/Care Manager.
  • Support clients with mobility aids, range-of-motion exercises, and non-sterile dressings, under the supervision of an RN.
