Responsibilities:- Liaise with clients, family members, staff, and stakeholders to ensure services align with clients' needs, goals, and preferences.
- Assess, plan, and review care plans, focusing on client-centric and consumer-directed services.
- Manage sales inquiries, develop new referral sources, and maintain existing relationships.
- Demonstrated knowledge and experience in coordinating care services for government-funded clients.
- Familiarity with relevant legislation and regulations.
- Advanced communication skills and ability to model a positive culture through professional service.
- Understanding of changing and diverse client needs.
- Reliable transportation and valid driver's license.
- Australian National Police check.
- First Aid and CPR certification.
- Experience in managing staff and developing new business opportunities.
- Possession of passion for high-quality client-focused care in a community setting.
- Exceptional communication and interpersonal skills.
- Ability to work autonomously or as part of a team.
- Excellent time management and organization skills.
- Commitment to ongoing professional development.
