Aged Care Clinician Vacancy

  • This position offers a unique opportunity to work in a dynamic team, delivering comprehensive assessment and care planning for older clients.

About the Role:

We are seeking an experienced ACAS clinician with a nursing or allied health background, to join our team on a casual basis. The primary objective of this role is to complete detailed individual assessments and develop personalized care plans for eligible clients.

Responsibilities:

  • Conduct comprehensive assessments of eligible clients to identify their care needs and develop tailored care plans.
  • Collaborate with multidisciplinary teams to deliver high-quality patient care and achieve optimal outcomes.

Skills and Qualifications:

  • Bachelor's degree in Nursing or Allied Health discipline.
  • Minimum 2 years' experience working in aged care, preferably in an assessment and care planning capacity.
  • Excellent communication and interpersonal skills, with ability to build strong relationships with clients, families, and healthcare professionals.
