Job Overview:
We are seeking an experienced and compassionate individual to join our team as a Comprehensive Aged Care Professional.
About the Role:
This is a unique opportunity to make a real difference in the lives of older adults, assessing their needs and connecting them with essential services that support their independence and quality of life.
Key Responsibilities:
- Conduct thorough assessments to identify the needs of older adults
- Facilitate referrals to vital services that promote independent living
- Promote the retention and maximization of function and capability in daily living
- Work closely with families and carers, recognizing their key role
- Ensure all assessments meet departmental requirements
Requirements:
- At least 1 year of experience in aged care, including client assessments and service delivery
- Tertiary education in a health-related discipline (e.g., nursing, occupational therapy, physiotherapy, social work)
- Current unrestricted General Registration with AHPRA or relevant professional association
- Proficiency in Microsoft Office applications, including Word, Excel, Outlook, and client management systems
- Current Australian driver's license, comprehensively insured vehicle, and ability to travel within service area
