Aged care services are a vital component of healthcare, aimed at enhancing the well-being and quality of life for older adults.

As an experienced clinician with a nursing or allied health background, you will play a crucial role in our team, working within the Community Aged Care Services Team to deliver high-quality care to clients. Supported by the Team Leader, you will be responsible for completing comprehensive assessments and care planning for eligible older clients.

The primary objective of this role is to provide individualized care and support to older adults, ensuring their needs are met and they receive the best possible outcomes. You will work collaboratively with multidisciplinary teams to develop and implement care plans that promote the physical, emotional, and social well-being of clients.
