As an Aged Care Assessment Service Clinician, you will play a vital role in helping us achieve this goal by conducting comprehensive assessments, providing information and referring clients to appropriate services, and working closely with our team to support program achievement of Commonwealth and State reportable KPI's.

About the Role

  • Conduct thorough assessments using the Integrated Assessment Tool and provide personalized recommendations for services that meet each client's unique needs.\
  • Build strong relationships with clients and their families, providing guidance and support throughout the assessment process.\
  • Collaborate with colleagues to ensure seamless communication and coordination, promoting a culture of teamwork and excellence.\
  • Develop and maintain expertise in areas such as gerontology, health promotion, and disease prevention, staying up-to-date with the latest research and best practices.\
  • Participate in quality improvement initiatives, contributing to the development of policies and procedures that enhance our service delivery.

Required Skills and Qualifications

To succeed in this role, you will possess:

  • A current registration as a Registered Nurse or Allied Health Professional, or eligibility for membership of a relevant professional association.\
  • Strong assessment and clinical skills, with experience in aged care or community settings.\
  • Excellent interpersonal and communication skills, with the ability to build rapport with diverse stakeholders.\
  • A willingness to learn and adapt, embracing new ideas and perspectives to drive innovation and excellence.\
  • Flexibility and resilience, with the ability to navigate changing priorities and demands.
