Job Description:

Clinical Aged Care Professionals play a vital role in delivering comprehensive assessment services to clients. Their expertise enables clients to access the care and support they need to live independently and safely at home.

As part of our dedicated team, you will be responsible for conducting thorough assessments, reviewing client information, and developing tailored support plans that cater to individual needs and preferences.

The successful candidate will possess excellent communication skills, enabling them to effectively engage with clients, families, and healthcare professionals.

Key Responsibilities:

  • Conduct thorough assessments to identify client needs and develop comprehensive support plans.
  • Review and update client information to ensure accurate and timely delivery of services.
  • Collaborate with multidisciplinary teams to provide holistic care and support to clients.
  • Monitor client progress and adjust support plans as needed.
  • Ensure compliance with relevant legislation, funding guidelines, and service standards.

Required Skills and Qualifications:

To be successful in this role, you will require:

  • Tertiary qualification in a health-related discipline directly related to aged care or a specialist area such as Allied Health or Social Work.
  • Current unrestricted registration with the Australian Health Practitioners Regulation Agency (AHPRA) or other relevant professional association.
  • Minimum of one year demonstrated experience delivering services in aged care settings and/or to aged persons.
