The Role

The Community Services team coordinates care and services for clients who are living in their homes, supporting them to live as independently as possible and connected to their community. The team oversees and supports an extensive portfolio of clients, through a diverse range of programs. Care and services are underpinned by a holistic assessment of clients' needs and the identification of their goals.

The Community Services Care Clinician acts as a clinical resource to the Support at Home Services Team (HCP) and provides clinical input to client assessment and care planning. This may include client clinical assessment, reviewing the provision of support to the team to identify client clinical risks, developing client care plans, and support to initiate referrals where required.

Requirements

  • Conducts comprehensive and accurate care assessments using appropriate assessment tools.
  • Conducts client clinical risk assessment(s).
  • Supports the development of comprehensive care planning documentation and/or recommendations, including a timeframe based on assessed needs.
  • Involves the care recipient and their family as an active partner in the process of care and provides person-centered care.
  • Operates in partnership with the older person, their representative, carers, and others, supporting decision-making in line with the person's needs, goals, and preferences.
  • Performs work in line with best practice, tailored to the person's needs, optimising their health and wellbeing, promoting reablement, rehabilitation, and independence.
  • Documents in accordance with policy, maintaining records that are appropriate and in accordance with aged care standards.

About You
  • Previous aged care experience and working with a Case Management Team
  • A strong commitment to excellence and quality in the provision of aged care related services with a sound working knowledge of quality principles and accreditation.
  • Excellent communication and interpersonal skills to ensure the achievement of quality outcomes.
  • Demonstrated knowledge of safe work practices.
  • Good time management skills managing competing demands within the scope of the role
  • Good interpersonal skills and empathy for the elderly.
  • Computer literacy.
  • Completion of documentation for desired audiences
  • Development of clinical care plans and risk assessments
  • Interpreting and offering guidance from medical documents e.g. hospital discharge summaries
