Job Description

The Family Carer Support Specialist will provide critical assessment, case planning, and collaborative support to families. This role will involve identifying barriers and implementing effective solutions to help carers manage their responsibilities.

This position is part of the Early Intervention Services (EIS) and reports directly to the Program Coordinator. The primary focus will be on providing in-home support and resources for carers to overcome the challenges they face when caring for their children.

Key Responsibilities:
  • Conduct initial assessments to identify family needs and develop a comprehensive case plan.
  • Provide tailored support and resources to help carers manage their responsibilities effectively.
  • Oversee case management activities to ensure that carers receive the necessary assistance and support.
  • Collaborate with community services and other agencies to deliver holistic support to carers and their families.
