As a professional committed to delivering high-quality support services, you will play a vital role in supporting patients and their families within the hospital environment.
About the Position- Deliver exceptional services including crisis intervention, psychosocial assessments, general counseling, advocacy, case management, therapeutic support, and complex discharge planning to address diverse patient needs.
- Facilitate seamless transitions and address emotional and social needs to promote patient well-being.
- Hold a degree in Social Work with eligibility for membership of a relevant professional association.
- Demonstrate an understanding of the psycho-social impact of unexpected hospitalization due to illness, accident, or trauma.
- Be capable of completing comprehensive psychosocial assessments and implementing effective discharge and care plans in an acute aged care setting.
- Possess experience working as a social worker in an acute aged care setting, including experience with NCAT, Guardianship Division applications and hearings, elder abuse, and complex family situations.
- Exhibit strong written and oral communication skills.
