Youth Health Professional with a Focus on Marginalized Youth

About Us: Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care to residents of the Greater Western Region.

We prioritise marginalised, homeless and at risk young people aged 12-24 years. This position will facilitate engagement by providing services that are relevant and responsive to the health needs of the target group.
Your Role: The Youth Health Worker works within a multidisciplinary team and provides services for young people aged 12-24 years and their significant others.
This involves the provision of information, assertive case management, support, group work, health education, brief intervention and referral to appropriate services both internal and external to WSLHD Youth Health.
The youth health worker provides clients with a consistent staff member who can actively facilitate and coordinate service access and care navigation within a primary health care (PHC) framework.
Key Responsibilities:
- Provide services for young people aged 12-24 years and their significant others.
- Facilitate engagement by providing services that are relevant and responsive to the health needs of the target group.
- Provision of information, assertive case management, support, group work, health education, brief intervention and referral to appropriate services.
- Actively facilitate and coordinate service access and care navigation within a PHC framework.
What You'll Need: Qualification: Tertiary qualifications in a relevant discipline Selection Criteria:
- Qualifications and/or experience in a relevant field.
- Valid NSW drivers licence.
- Experience working with and understanding of the issues affecting marginalised young people's access to health services.
- Demonstrated experience in planning, delivering and evaluating health promotion activities and/or community development projects.
- Knowledge of the health issues that impact on marginalised and at risk young people aged 12-24 yrs.
- Excellent communication and networking skills.
- Demonstrated ability to work as part of a multidisciplinary team.
- Excellent computer skills with experience in a range of Microsoft applications.
- Demonstrated ability to engage with marginalised and at risk young people both individually and in a group setting.
Salary Range: Dependent on Qualifications ($85,000 - $115,000 per annum)
